I have this orchid I purchased several months ago. since the blooms have fallen I have had no hint of any more coming. the top of stem turned bit yellow and darkened a month or so ago...so I nipped that off to next section. now I see that another 2" have turned yellow and stem is dying. see pic
I have several air roots but one has turned black on the tip. see pic

it is in se window not in direct sunlight. I do not over water it. don't know what I am doing wrong.

Your orchid seems to be doing what orchids do, bloom for awhile and then take a rest. The flower stalk dies and you need to wait for another to grow from the base. As an epiphyte some plants will grow adventitious roots for support. You are probably not doing anything wrong. The leaves appear to be healthy so keep to your watering schedule. The hardest part is waiting.

My Phal orchid rebloomed 3 times on the same stalk.

The roots probably need more air than they can get in that pot, especially if the pot is full of moss &/or soil.
